STEP 1: Learn about NASH Services

Call us toll free (833) 833-6204 or use our online form to contact us and learn about your employer’s specialty pharmacy program. Tell us what medicines you take and what insurance you have. Our team will check whether you are eligible. Just say, “Hi, my name is [Name] and would like to enroll as a new patient for NASH services.”
STEP 2: Submit New Patient Forms
All new NASH patients must enroll and provide patient information and consent. Click to access the Patient Information form in English or Spanish. Fill out the information requested and click “submit” at the bottom of each page.

STEP 3: Get Your First Fill
A member of the NASH team will review your information and get in touch. If you qualify, NASH will send your medication right to your home (or work). Just a signature is required for home delivery!

NASH keeps track of when you’ll need more medication and helps you order refills on time.
