STEP 1: Learn about NASH services
Our Patient FAQ may answer many of your questions. But, if you still have questions, contact us!
Call us toll free (883) 883-6204 or use our online form to contact us and learn about your employer’s specialty pharmacy program. Tell us what medicines you take and what insurance you have. Our team will check whether you are eligible. Just say, “Hi, my name is [Name]. I work for Four Winds Hospital and would like to enroll as a new patient for NASH services.”
STEP 2: Submit New Patient Forms
All new NASH patients must fill out the three forms. Click each link below to access the forms in English or Spanish. Fill out each form, and click “submit” at the bottom of each completed form.

STEP 3: Get Your First Fill
A member of the NASH team will review your information and get in touch. If you qualify, NASH will send your medication right to your home (or work).
Ongoing: Get Your Refills
NASH keeps track of when you’ll need more medication and helps you order refills on time.
